Para-aminohippuric acid (PAH) is a compound used in renal physiology to measure renal plasma flow (RPF), an important indicator of kidney function. PAH is filtered by the glomeruli and also secreted into the urine by the renal tubules, making it a useful marker, as nearly all the PAH that enters the kidneys is excreted. Measurement of PAH clearance is based on the principle that its rate of excretion is proportional to the plasma flow through the kidneys. This allows for the calculation of effective renal plasma flow (ERPF), which is an estimate of the actual plasma volume passing through the kidneys per unit of time. Although not routinely used in clinical practice due to the advent of newer, less invasive methods of assessing kidney function, PAH clearance remains a gold standard in research settings for accurately measuring renal blood flow.
